AOSC Cancellation Policy

• For any procedures scheduled at Arkansas Outpatient Surgery Center, we now require a 48 hour notice prior to any cancellations.
• If you do not give a 48 hour notice of cancellation you will be required to pay a $100 cancellation fee. You will not be seen until this fee is paid.
• You MUST show up to your appointment at your scheduled check in time. If you are unable to show up at your scheduled time at you WILL BE required to reschedule.
• Checks are not accepted at Arkansas Outpatient Surgery Center under any circumstance.
• NO EXCEPTIONS. Please, bring this signed copy with you to your procedure appointment.

The surgeons in this practice have some ownership in Arkansas Outpatient Surgery Center. We respect the rights of our patients to choose not only their surgeon but also where they wish to have their surgery or procedure performed.
